Dental implants are the most comfortable treatment option for partial or complete tooth loss. However, after tooth loss, especially following the loss of all teeth, the jawbone loses height and width. In cases where this loss is severe, meaning the jawbone volume is greatly reduced, fixed prostheses can be made using 4 implants in the lower jaw and 4 implants in the upper jaw. This procedure is called All-on-Four. On the day the implants are placed, a temporary, fixed, and immediately loaded full prosthesis is prepared and placed on the implants. The surgical procedure is performed as with all implant surgeries. The only difference is that the implants placed in the posterior region are positioned at a different angle.

In cases of complete tooth loss, due to bone loss in the jaw and the reduced distance to the nerve canal in the lower jaw, or the downward displacement of the sinus cavities in the upper jaw, implants may not be placed in these regions. In such cases, the only option for fixed prostheses may be implants placed only in the anterior region using the All-on-Four technique. Most full denture patients cannot fully perform chewing functions over time, especially due to the weak retention of the prosthesis in the lower jaw. Fixed prostheses made possible by the All-on-Four procedure allow the patient to chew comfortably and without pain. Additionally, the implants prevent further bone loss in the jaw, preserving the bone tissue. Since the stability of the prosthesis is provided by the implants, patients do not need to use larger prostheses.
In cases where bone tissue has severely decreased, surgical procedures to augment the bone increase the risk of complications over time. With the All-on-Four procedure, minimal surgical intervention reduces these surgical complication risks.
Patients can receive both their implants and prostheses on the same day, completing their treatment without ever being toothless.
Advantages of the All-on-Four Treatment Concept:
Angled implants placed in the posterior region of the jaw prevent damage to anatomical structures.
Angled posterior implants are longer and placed in higher-quality bone.
There is no need for bone graft procedures.
Allows for prostheses that are easier to clean.
Immediate esthetics and function are achieved.
Further bone loss is prevented.
Speech and chewing functions are improved.
Healing occurs in a short time with less pain and discomfort.
