Orthodontic tooth extraction is rarely necessary in some cases. In certain patients undergoing orthodontic treatment (though this number has decreased over the years with new techniques and treatment philosophies), it may be necessary to remove some primary teeth or, in other cases, permanent teeth at specific timings.

The purpose of tooth extraction is generally to better preserve the overall oral health of the remaining teeth. In recent years, extraction planning is not performed for any patient where treatment can succeed without it.

Tooth extraction is now almost a rarely recommended treatment. However, if recommended by the orthodontist, not extracting the tooth would pose a risk to the health of the other teeth. In such cases, sacrificing one or a few teeth is preferable to jeopardizing the health of all teeth. If there are alternatives to extraction, the orthodontist will explain the treatment options to you.

To prevent unwanted tooth movements and stop interference with adjacent teeth, extraction may be necessary. Orthodontic treatment should ideally begin immediately after extraction. In some cases, treatment may not need to start immediately if additional procedures are required in the extraction area, as healing time must be allowed. The treating orthodontist is the best person to determine the appropriate method and timeline for these situations.

WHY IS TOOTH EXTRACTION SOMETIMES NECESSARY IN ORTHODONTIC TREATMENT?

  • To resolve crowding in the jaw,

  • If there are more teeth than should be present in the jaw and these extra teeth prevent the healthy development of other teeth,

  • For teeth that threaten the alignment of healthy teeth and cannot be corrected, extraction may be necessary.

WHY TOOTH EXTRACTION IS DONE IN ADULT ORTHODONTIC TREATMENT?

  • To create space for proper alignment of the teeth,

  • Extraction of partially impacted wisdom teeth that are difficult to maintain,

  • Extraction of impacted wisdom teeth (if not removed, these teeth may cause problems later),

  • Extraction of previously undetected impacted teeth in different areas of the jaw.

Every tooth is important, but the priority is maintaining a healthy oral and dental condition. Extractions should be performed under the guidance of the orthodontist when necessary.
